Your first appointment

For your appointment, we ask that you bring the following:

Your GP introduction
X-rays, MRIs, CT scans, ultrasound or other images you may have (and image reports)
Copy of operating records and medical records
If you have met a physiotherapist, please bring a letter of advance from your physiotherapist.
Medicare card
Details about private health insurance
If you are an employee of the Declaration of Employment or TAC, please bring the claim number, full name of the case manager, contact details (including telephone number, fax number and email address) , date of accident and related documents.
Victorian Patient Transportation Assistance Program

If you live in the countryside and the Victoria area, see a health professional who may require a lot of travel and you may also need accommodation.

The Victorian Patient Transportation Assistance Program (VPTAS) can provide financial assistance for related costs. VPTAS provides financial assistance to eligible patients living in rural and remote areas of Victoria for access to specialized medical services.

In order to receive a subsidy, you need to complete the VPTAS Claim Form.
